I'm a green card holder and at the process of petitioning for my wife who lives in our home country, by filling form I-130.

1. The question is do i need to file form I-130A as well, have called USCIS twice but there were unable to tell me which forms to include in the package.

2.on form I-130 on part 4 question 59.A, ''if we have lived together before" , she stayed with me last time she came to the US for like 3 months and go back to her country, do i need to type ''NEVER LIVED TOGETHER'' or not.

3. do i staple the package together or use paper clip.

Yes, Check the last page of the i130 instruction where it says the checklist.

You lived together for 3 months, so better to put that address where you lived together even if it's temporary.

No staples, use paper clips

I would still put it, you two lived together for 3 months. It doesn't hurt to put that since you two last lived together even though for a short time. I did same for mine.

2015-08-17: I-130 approved

2015-09-23: NVC received file

2015-10-05: NVC assigned Case number, Invoice ID & Beneficiary ID

2016-06-30: DS-261 completed, AOS Fee Paid, WL received

2016-07-06: DS-260 Submitted

2016-07-07: AOS and IV Package mailed
