German Police check

Good day all, 

Looks like I will possibly have to do a German Police check. I have the forms downloaded - it says on the bottom

From looking at this can I go to local Police station or will that not be acceptable??  If not has anyone else applied and who/how did they get it signed and how much did it cost????
