Filling address in form g325a for beneficiary

Hi everyone. I am filling for my form g325a. I am confused how to fill my Vietnamese address accurately. Please help me!. 

First, my residence in last 5 years, i have been living at my parents house since 2008 ( about 9 years). Before this time, i lived in different place. Should i include both of these address ? 

Second, address of employer, my company's address is 123 lot A Lac long quan residence tong van tran street ( It's too long and i need an extra space) Can i just write " 123 lot A lac long quan residence ??
