If you want to fill up a DS-160 yourself, you need to go to this link https://ceac.state.gov/genniv/  :) The DS-160 can only be completed online.

If you are asking for the print view of a filled DS-160 sample, check this out http://www.visajourney.com/examples/DS-160-Example-Form.pdf

As for the barcode, I am not entirely sure but perhaps you could provide the full sentence concerning the barcode that was mentioned in the Canadian packet and I'll see if I'm able to explain it to you :)

For my I-129F, K-1, AOS, EAD, AP and ROC detailed timelines, please refer to my timeline page :)

ROC filed on December 1, 2020, assigned to SRC, approved within 106 days on February 18, 2021.

Thank you so much. I kinda thought the application had to be submitted online. Wasn't sure why my attorney had their own printed out with their latter head on it. From what I've been researching it looks like once you get onto the application it gives you a barcode if I'm understanding right. This seems very confusing to me, :(

Once you have completed the form, reviewed and signed it online, you will get to a confirmation page screen for you to print that page out. On that page, that is where the barcode would be :) You'll need to bring the confirmation page with you to your interview at the embassy.

Just put something tentative like 3 months from now. That tentative date won't be used against you. You can still travel at a different date than stated in the DS-160 as long as you travel to the US within the 6 months of a valid K-1.
