Need Help Wrapping Up Application

My wife and I are almost ready to submit our adjustment of status application. She is a USC and I am adjusting from DACA. We have almost all documentation ready and the app almost completed but have nitty gritty questions that we need help on. Can someone please help clarify the following?

 

1. I have birth certificate in Chinese that I got from the consulate's office in the States mailed to me. They are regular paper copies but have stamps on them. Does this count as an original?

 

2. The Chinese consulates site has birth certificate templates in English for easy translation. You pretty much just have to change the highlighted text in the template that is specific to your information. For example, you just substitute your name, address, DOB, parent's name, etc. in the highlighted fields. Is using this template ok? 

 

3. How does the translator's attestation need to be? Can I just have the translator write on the back of the the translate birth certificates in pen and sign? Or, should I type it up and just staple the attestation to the translated certificate?

 

4. I have most of my vaccinations. However, there are some vaccinations I haven't started or can't find records of. To complete the vaccination entirely, I'll have to wait because there is a waiting period between each dose. Can I still submit my physical and application even if I have just 1 dose of the series so far OR do I have to wait until the entire vaccine series is complete which can delay me for half an year? If I can submit after getting just my first dose, how do I submit proof that I completed my vaccination series? Do I mail proof of the 2nd and 3rd dose? Do I bring the copy of the vaccination to the interview? Or do they not care?
