Fiance is applying for AOS as K1 visa holder.

Arrival date was 1/26/17

As I understand it, the civil surgeon medical does not need to be completed in order to apply for AOS.  It only needs to be completed prior to the AOS interview.

Is this a correct statement?    I ask because I see the I-693 form listed on the AOS checklist here at Visa Journey.

My fiance did have to do the sputum testing prior to receiving her visa.   Does that affect the situation?

Do you have a copy of the DS3025 to send in with the I-485 packet?  Is it marked complete?  If not, then the I-693 will need to be completed by civil surgeon and sent in with the I-485 packet.

We did not get a DS3025.

The Medical in Manila only provided a vacination "worksheet".   Nothing else.

I have contact the US embassy in manila to try and get a copy of the medical records but they refuse to do it.

I do not understand how we cannot get copiies of her own medical records that were used to get the K1 visa in the first place.

p.s I called the USCIS and they told me that the medical did not have to be done to apply for I485.

Is there any number to call where questions can be answered?  I tried and they refused to answer any questions about forms.   Unbelievable.

You do not get a copy of the medical records.  You are entitled to the DS3025, vaccination sheet.  I made sure my husband didn't leave the medical facility without it.

I believe the medical vaccination worksheet is the DS3025.  Do you see that number on the worksheet anywhere?

I would make a copy of that and send it in with your AOS packet.

If your DS3025 was marked complete then no need for the I-693 at all if you file within one year of the medical date.
If you have a copy then check if it's marked complete, if yes do nothing, if no you need to have the I-693 either with

your AOS package or submit it with a RFE you will receive. The RFE can come at any time or at the interview but not

only at the interview.
If you don't know if it was marked complete or not then wait to see if you will receive a RFE.
