Help! AOS no DS-3025

Hello Vjr's! i think I'm screwed, I'm working on my AOS paperwork and i realize the physician never gave me a copy of DS-3025. What should I do? I'm adjusting from K1 visa. Anyone faced that situation before ? Thanks for your replies!

It's an important paper to ask for when you have your medical.

 

Can you contact the clinic and ask if they can send a copy via mail or email?

 

Otherwise you have to take your vaccination records that you used to prove what vaccinations you've had in your life at your medical, and bring them to an approved panel physician here in the US and try to get him/her to fill out the vaccination part of form i-693 and sign it. Then you can add that one to your AOS package instead of the DS-3025.

Thanks scandy. I remember the physician filling the medical file with a pen not on  on the computer and he never made a copy of it.  So I'm pretty sure he doesn't have any record of my medical file so my best option would maybe be to take my vaccination records that are used for the medicals to a panel physician here and Make him sign that for me but I don't know how much that will take. Do you guys have any idea?

Read through this pinned thread it will give you the answers you need.
In a nut shell, if you know your DS3025 was marked complete do nothing.
If you do not know if it was marked complete then wait to see if they send a RFE.
If you know it was NOT marked complete then a visit to the CS is needed.

Thanks Ebolonuwa!! I know it was marked complete. I contacted my physician abroad he told me

he will send a copy via email he's still in vacation. So I'll go ahead and file without it, and if I ever get an rfe, I'll be having that ready .

We are trying to help determine the validity of our DS-3025. This is question is specifically referring to the "Influenza" portion. My fiance (Brazil) had her medical exam in Brazil; we have the DS-3025 showing (with dates) she received DT, DTP, DTaP, OPV, MMR, Measles, Other MCV conjugate, and Varicella History is checked. For Influenza, however; in the blanket waiver column she has the letter A. At the bottom the US Vaccination Requirements box is checked. The doctor's signature and stamp are at the bottom.

 

I am under the impression we do not need the I-693 form. But, regardless if we do or do not, is one needed to be submitted, even blank while also enclosing the DS-3025?

 

Lastly.. we've been married about a week and a half and are now looking into the above as well as AOS at this time. If there's anything else pertaining to our situation it seems like we are forgetting, we'd so greatly appreciate your advice. Many thanks, VJ community.

If the medical was outside of flu season no flu shot is needed. As long as a letter or number is in the waiver/column you are ok.
From what I read in your post no I-693 is needed.
