AOS filing on K-1 Do We Need to File I-693?

Hi, I am new to this forum.  My fiance is from Canada on a K-1 visa and we were just married a week ago.  We are preparing to file an AOS (Adjustment of Status) but have a question regarding the need to also file the I-693 (Report of Medical Examination & Vaccine Record).   My fiance had taken his physical exam in December 2016 and we have the DS-3025 stating that all required vaccinations are complete.  Do we sill need to fill out and file the form I-693 as part of the AOS package even though the exam took place just over three month ago or do we only need to include a copy of the DS-3025?    Also, if we do need to submit the I-693, does that mean we need to go to a civil surgeon to sign the form even though a new physical exam is not required?

 

This is very confusing.  I have not found a clear answer yet on this.  Your help is great appreciated.  Thanks!

According to the instructions, you can just file with the DS-3025 if the original medical has not passed one year. However, some people got the RFE for not submitting the I-163 supplement a.k.a records transcribed by a civil surgeon. So maybe play safe and get it transcribed for submission anyways?

ROC filed on December 1, 2020, assigned to SRC, approved within 106 days on February 18, 2021.
