Question about filing I-130 and I-485 concurrently

We are trying to file I-130 and I-485 concurrently for my father. We had few questions, if anyone is able to help us:

 

1) Should be include separate check for I-130 ($535) and I-485($1140 + $85). Should be combine I-485 fees with biometrics ($1225) or separate?

 

2) We are going to include G325a for father. Do we need to include G325a for applicant? Also some of the details of father's parents (birth date and place) are unavailable due poor record keeping in our country back in the days, should we just say unknown on G325a?

 

3) I-94 had slight error in the spelling of first name (one character- came out X as opposed to K). Where it asks to put name exactly as it appears on I-94? Should be follow I-94 or correct spelling?

 

4) We are also gonna include I-864 and Medical Form. Anything else we need to include?

 

In terms of supporting evidence:

1) My birth certificate

2) My father's birth certifcate

3) Father's birth certificate

4) Proof of citizenship for me

5) I-864 supporting docs

6) copy of I-94
