What documents are needed to prove you are a US citizen when filling out the petition? I am using passport but I came confused as to if it is asking for a 2nd form of proof. And how do you write out a Jamaican address on the form without a house number?

a birth certificate would work. When in doubt explore the instructions to the corresponding form straight from USCIS.gov

 

Instructions say this:

What Documents Do You Need to Show That You Are a U.S. Citizen?

1.If you were born in the United States, submit a copy, front and back, of your birth certificate.

2.If you were naturalized, submit a copy, front and back, of your original Certificate of Naturalization.

3.If you were born outside the United States and you are a U.S. citizen through your "parents", submit:

    a(1) A copy of your original Certificate of Citizenship; or

    b(2) Your Form FS-240, Report of Birth Abroad of a United States Citizen.

4. In place of any of the above, you may submit a copy of your valid, unexpired U.S. passport issued with a validity

period of at least 5 years. You must submit copies of all pages in the passport

also (because the site won't let me edit) I wouldn't sweat too much if the address isn't how our postal addresses are here in the states. There are many here whose countries have a crappy or nonexistant postal service. My husband's Nica address was like "Road to Masaya, 25 varas to the south in the Primavera neighborhood (something like that). USCIS sent me all the important documents anyway and he ended up having to pick up his passport in a specific courier service anyway.

 

I'm sure someone from the Jamaica portal can let you know how exactly the address thing would work for you

Birth certificate is always first choice to submit, then the entire copy of ur passport.  

 

His address is OK to write it as he says. They do not have zipcodes.  They have parrishes
