Hello, 

 

1. I know immunization questions get asked all the time, but I searched and couldn't find answers to my particular questions. My fiancee had her medical exam in Vietnam, prior to her interview. I was wondering if she still needed to see a civil surgeon for form I-693; in the stickied topic on this forum, it says, "If your DS-3025 from your visa medical is sufficient to prove you have completed all required immunizations, then use it instead of seeing a Civil Surgeon." Then later it says:

"What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it"

 

But my fiancee got her shots AFTER her medical exam, so I don't think there is any evidence of the vaccinations within the medical results that will be turned over at POE. Does this mean she would still need to see a Civil Surgeon to get the immunizations transcribed onto the I-693? 

 

2. She had the first dose of Hep B, but missed the second because she couldn't get off work to go back to Saigon. She wants to go back now to get it (being a little late shouldn't matter, from what I've read), but it's a 6 hour bus ride each way, and I don't want her to waste her time if she doesn't need to. From the age chart I've seen, someone over 18 shouldn't even be required to get Hep B; so I'm wondering why they gave it to her anyway? But regardless, she should be okay to apply for AOS without the 2nd and 3rd shots, right?

 

3. This may be moot anyway, because I'm not sure she got DTaP; I was with her when she got the vaccines, and I specifically remember being surprised they didn't give her that one, and told her to ask about it. She asked and they said they didn't have it. Since it's certainly required, if she can't get it before she moves here, is it possible to get it from a clinic, pharmacy, etc., assuming we can find a Civil Surgeon who is willing to just transcribe vaccine records for a fee? Would he take the records from Vietnam and any new ones from the US and do the transcription?

 

Thanks for any help! It was nice to have a little break after the visa interview, but now I've got a feeling the real work is about to begin.

If she has a copy of the DS3025 from the panel physician and it is marked complete (little box in the bottom quarter of the page) she needs to include a copy of that with the AOS paperwork.  If she doesn't have that or the DS3025 isn't naked as complete, she will need to see a civil surgeon.

 

TDAP is required and should be available at most clinics as it is a standard vaccination.

 

Hep B is not required.  I was erring on the side of caution and got a whole bunch of the vaccinations (plus healthcare is free in Canada), so I had the first hep shot before my medical.  The panel physician went ahead and didn't even note that.  He filled out the dates for all the absolutely necessary ones and asked for a waiver for the rest as "not medically appropriate."

 

Yes, have a little break, but more

work to come, especially if you want to file AOS/EAD/AP as soon as possible.... but then, life will normalize :-)

It doesn't say anything about DS3025, or anything marked as complete, or waiver for not age-appropriate or anything like that...so I'm guessing this has nothing to do with the DS-3025 form? And since all of her vaccines were AFTER her medical exam, I don't see any way a completed DS-3025 could be in her file anywhere. So that leads me to the only other question I have right now: I found a Civil Surgeon near me who apparently charges like $50 to transcribe immunization information onto form I-693. Is it safe to assume I can take my fiancee to any clinic, pharmacy, whatever that gives Tdap (which should be free as soon as I add her to my insurance), and then take that Tdap record, along with this immunization paper, and the other medical information from that packet, to the Civil Surgeon and he will take care of the I-693? Am I understanding things correctly? Thanks!

Hmm... that's weird that the panel physician didn't include a copy of the DS3025 for her records.  But perhaps bc she wasn't completed, it didn't make sense as the I693 would be required for AOS.

 

But yes, you'll take that immunization record and the proof  of TDAP to the civil surgeon.  Not

entirely sure if they'll re-do anything from the original medical (in terms of examination) but all that's required is the vaccination supplement. 

 

I'd double check the fees of the civil surgeon bc $50 seems unbelievably reasonable.  I hope that's correct for you, but when I was looking the cheapest I could find was $250.00, but then I found the 3025 so I didn't have to go.  

 

But definitely dont have her spend an exorbitant amount of time on a bus cause she can do that here.

Awesome - thanks again. Yes, I found a thread somewhere on here where people list Civil Surgeons who charge reasonable fees just to transcribe the vaccines if no medical exam or vaccinations are needed. The thread is a few years old, but hopefully that's still the case!
