No. The one you sent was Document Cover-sheet, which is to be on top of both of your packages. "Document Cover Sheet"  also shows the order of documents.

 

AOS and IV Cover Sheets contain Bar-code  on them. Please follow the steps and see. If you receive the same Cover sheet, you may want to contact NVC.

Yes. This is the one. Once you pay IV fees, then you will be able to download IV Cover sheet as well. And then you can send the documents.

NVC told me that I only needed to include one of the cover sheets, and that the important thing was to include one of the cover sheets with a bar code. Don´t rely on me, but based on the information I got I only included one of the cover sheets with the bar code (chose the one with the check list).
