Hi,

I've read through and read about the immunizations and I think I'm clear on what I need to send, but just want to make sure.

 

My husband had his medical done in London by a USCIS approved civil surgeon. At the time he was told he needed an additional 3 vaccinations, but he opted to get them done by his GP because it was going to be $200 cheaper there than in London. He got them done, had them recorded, brought them with him. We pulled the required sections needed to get them transcribed, but it seems like NONE of the civil surgeons we talked to knew what to do. They all wanted to do a full medical AGAIN, despite us showing them the instructions on the 693 that because he came over on a K1 visa, and his medical was done within the year by an approved physician, he didn't another. We finally found one willing to transcribe his immunizations. They did so, then gave us a copy and sealed a copy in an envelope and told us to send that envelope to INS, and not to open it. I know we just need 4 sections filled out, so is it okay to just send those, or do I send the entire form that only has those sections filled that is in the sealed envelope?
