Use your legal name. If you changed it when you were married, use it.

Claudeth used her married name, it just seemed to make it easier. She did have to get her passport renewed and that was a whole bunch of fun dealing with the Filipino embassy in L.A. Her new passport is now in her married name too so everything matches.

Not everyone uses their married name...that could be the reason she was asking.

Sorry!! I did get married, but I was just not sure if when filling the forms they would want my birth name or married..When they do the name check threw the FBI, my married name is new and they would find nothing based under that name..Thats why I am alittle confused!! But thatnkyou for all the responces everyone!!! I guess I am going to use my married name..I hope that well be ok...

That will be fine. Like I suggested before go to the Example Forms link and see how others filed, made my filing of forms much easier. Also on most forms it will ask what other names you have used and they will look up your maiden name when they do the fbi check. Good luck,
