MAIDEN NAME VS MARRIED NAME

HELLO,

 

I'VE BEEN MARRIED FOR THREE MONTHS NOW, IAM APPLYING FOR AN ADJUSTMENT OF STATUS AND  WHILE FILLING ALL THE FORMS  I RAN INTO A PROBLEM: I WANT TO HAVE MY MARRIED NAME ON THE GREEN CARD SO THERE FOR I UNDERSTAND I HAVE TO PUT MY MARRIED NAME ON ALL THE FORMS, BUT WHEN I WENT TO GET MY MEDICAL EXAM I GAVE MY MAIDEN NAME, WHICH I CAN NOT CHANGE ANYMORE SINCE THEY GIVE IT TO YOU SEALED, IS THIS GOING TO BE A PROBLEM IF I HAVE MY MAIDEN NAME ON FORM I-693 AND MY MARRIED NAME ON THE REST OF THE FORMS??

Thank you Lainie, but my medical exam or form I-693  comes with my maiden name and had it done after I got married, it was my mistake since I'm not used to using my husband's last name but I really want to have my husband's or married name on the green card, you think it would affect or create confusion?
