Good afternoon, 

   I am completing the I-864 Affidavit of Support and am wondering what name I should use to complete the sponsor section.  I recently got married and changed my last name.  However, I haven't gone to the Social Security Office nor the DMV to make the change official.  This change has been made only on the marriage certificate which I just received yesterday by the way.  I want to know if I should use my NEW MARRIED NAME (although I have no documents that reflect this change) OR if I should still use my old name (although ALL of my state ID's, passport, SS Card, and bank statements show this name) ??? It sounds like a silly questions but I want to make sure I fill out the paperwork correctly. Thank you!

Use your married name, as that is your new legal name.  

 

Your marriage certificate is a document that reflects the change :-)

 

I can't remember the specifics of that form, but if it asks you to list former names, be sure to list your maiden name. 

 

And if you're anything like me, throw in a letter explaining you got married and assumed your husbands surname to remove any doubt they may have :-)

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.
