Hey Everyone,

My fiance had his medical done, and when he gave the sheet to the embassy, they told him the doctor had messed up the form, he'd put just all the shots he'd gotten for the visa, not as a child. So they made him redo the sheet at the last minute, and he was unable to get a cope. He does have all his recent and childhood shot records, and a copy of the sheet the doctor messed up on, just not the original. Any idea of how the civil surgeon will work with all that? And what is the typical cost for the doctor to fill out the supplemental sheet as my fiance has already had his exam in the past year for his K1 visa?

FYI the Civil Surgeon that I went to didn't accept the CDN Dr's worksheet at all.. He said "why would i take some stranger's word?". He wanted to see my documentation from Travel Clinics and my childhood innoculation record.

IT should be noted that this was the first time my particular Civil Surgeon had ever been asked to fill out the form, so I'm not sure he was 100% aware of the whole process. BUT I would bring as much info as you have (they are looking for specific dates the vacinations were done - keep that in mind).

We paid 25$ for ours. DON'T go to a Surgeon that will charge 100$.. keep in mind it's often extra cash for these Dr's and they pocket the money. Don't go to someone if you feel it's a rip off. Keep calling the listed Surgeons.

The above poster is correct, don't let the doctor give you a full medical again, it isn't necessary and don't bother wasting money on it. I paid only $5 for my I-963a, so really phone around, lots of them are going to say to get the full medical done though so be prepared. You only need the I-963a done. :thumbs:

IT should be noted that this was the first time my particular Civil Surgeon had ever been asked to fill out the form, so I'm not sure he was 100% aware of the whole process. BUT I would bring as much info as you have (they are looking for specific dates the vacinations were done - keep that in mind).

We paid 25$ for ours. DON'T go to a Surgeon that will charge 100$.. keep in mind it's often extra cash for these Dr's and they pocket the money. Don't go to someone if you feel it's a rip off. Keep calling the listed Surgeons.

QUESTION:

WHERE IS A VALID UP TO DATE CIVIL SURGEON LIST AT ON WEB??? I have checked USCIS and it is Error 404 and so is the one at Civil Surgeon website, just curious. ANY Info you can share would be greatly appreciated.

Look on the second or third page of this forum...someone posted the Civil Surgeon finder yesterday.

I just got my I-693A done today. $45 bucks, took a half an hour, they sealed a copy and gave me one for my records.

I didn't need any new vaccinations, but they did want to know the exact month and year that I had chicken pox. I had to call my mom, hahahahaha!

My local USCIS office had a list. I called around, some did want 100 but many did not even do any immigration medicals anymore. The one I did find across town was 25 dollars so I hurried on over there to find out that the government has the wrong address. Luckily the other doctors office that was there was able to look up his address and direct me. So, ensure you have the right address when you find an office that is reasonable. I had the form that the canadian CS filed out but they didn't have my hep b shots on there so luckily I had brought my card from when I got immunizations as a child. they however did not give me a copy of that record they created.

Oh well...they charged $150 for ours, but that included the blood test. The medical records that included all the immunizations were turned in at the K-1 interview stage, so my now wife had to get 2 other shots that cost another $65. There were only a couple of local doctors who were on the "list" oddly enough, and no doubt it's free money for them, but it was convenient. Quite a pain and quite a ripoff, too. Incidentally, this was for a doctor in California....just fyi.

We paid $25 for the vaccintation supplement. The doctor in France did not put that my husband had the chicken pox as a child on the DS-3025 copy that we received. So, my husband went to his primary care doctor here through insurance and had a blood test to show antibodies to chicken pox and we took this along with the DS-3025 to the civil surgeon here to complete the I-693a.
