Timeline

I didn't need any forms, other than my marriage certificate. You don't have to file any official form to register to the fact that you are married and changing your last name. For example, I had to change my name on my Canadian bank account -- I wasn't allowed to do so until I could show up to my bank branch in person with a certified copy of my marriage certificate and photo ID. Anything else that I applied for (like AOS, EAD, AP, etc), I applied for in my married name, indicated on the forms when asked that I used to be known by my maiden name, and showed the marriage certificate and photo ID to confirm.

make an infopass appointment and tell them you wasnt to change your last name..... they will advised the best way this can be done....

USCIS can't help you to legally change your name. You need to petition the local superior court with jurisdiction over your residence to change your name. There will be filing costs involved. In my area, the cost to file for a name change is $475.

To get your name changed on the adjustment of status application, you need to present a court order or a marriage certificate showing a name change. If your name wasn't changed on the marriage certificate, then a court order is the only way.

dayum in texas it is 180.00 < roughly > give or take a few dollers.
