Long Form Birth Certificate

I would imagine that my fiancee's "long form" birth certificate from Ontario will be needed somewhere down the line, correct? Or is the "short form" without the parent's names on it acceptable?

We've sent out for her daughter's long form birth certificate, finally, after finding out we needed that for her passport (as well as her daughter's father needing to sign the passport application, as well, even though it clearly states on the form that either parent may apply).

The long form birth certificate is required for your fiance and her daughter.

It's required at the consulate level. They will not accept just a short form, only the long form for all applicants.

Absolutely have to have the long form!!! We made a mistake and did not have the form at our interview. Luckily, we were able to run over to the Dept. of Vital Statistics and get a copy, otherwise we would not have got the visa that day!

HUH?? Mine is one sided, says certified photostat and has the stamp at the bottom of it.

Just an aside about the passport.... it says either parent may APPLY, it doesn't say anything about only one parent signing. Sorry I didn't see anything earlier about you guys apply for the little one's passport. I could have maybe helped out with what you needed.

I just applied for my long form online. Here's hoping it goes smoothly.
