That's what I thought too! But I just found out that there are two cover pages. The checklist one and another cover page  called "document cover page" that you have to print from the confirmation page with the AOS and IV fees. ? it has your name on it, the NVC address and another barcode. I didn't even think about another cover page since the checklist page clearly says Cover Sheet. I thought there was just one. Ahhh why.

Timeline

I sent my documents in Dec and got confirmation from NVC both by phone and email that the documents were received,

yesterday I received a response to the inquiry I did 2 weeks ago and it says,

 

"We cannot review your application until we receive an Affidavit of Support (Form I-864) and your supporting civil documents. If you already sent them to NVC, please disregard this letter."

 

such a stupid response after 2 weeks! Hope they didnt lose our documents after 9 weeks of waiting!

 

After 8 weeks, I wrote to congressman and his office did an inquiry on Feb 15th, hopefully this will do something to speed up :/

Still receiving "Database error". It has been 2 whole days since we are receiving this error while trying to pay IV Fees. Might there be a problem with the payment system? Anyone receiving this error please update when you're able to pay again.

 

The most frustrating thing is, we can't get any answers until Monday (not that CSR at NVC will give anything other than standard "we are uploading IV fee Bill for your case. It will become available soon", but there's still hope of getting correct answer)

06/24/2019 -- Card was charged with $680 fees. Text Message received confirming Receipt. Case routed to NBC (Case Number starts with 'MSC')

06/28/2019 -- NOA Hard Copy & 18 Months Extension Letter Received in Mail with NOA Date as 06/19/2019

07/12/2019 -- Biomentrics Appointment Letter received. The Letter is Dated 7/5/2019. Appointment is on 7/22/2019

07/22/2019 -- Biometrics Appointment Complete

02/01/2022 -- Interview Scheduled for 3/3/2022 (combo interview)

03/03/2022 -- Interview Complete. No new Green Card will be produced as my Naturalization was also approved.

Thank you for the link, but it says only about Applicant's civil docs, nothing about "Petitioner's Civil Docs". I have all the documents from my side ( beneficiary/applicant) and we are wondering if we need any civil documents from my wife (petitioner).

Petitioner documents are required if petitioner has been married previously. The documents must show that all previous marriages ended lawfully (either through divorce or death of former spouse) 

 

From NVC website: 

 

 

If you are applying for an IR1, CR1, or F2A visa as the spouse of a U.S. citizen or Lawful Permanent Resident and your petitioning spouse was previously married:  You must obtain evidence of the termination of EVERY prior marriage your petioning spouse has had. This evidence must be an original or certified copy of one of the following documents:  FINAL legal divorce decree, death certificate, or annulment papers.

Passport photos are no longer required for the NVC but you must take them to the interview.

That I did read. Thank you. We already have that. I was wondering about that because we were not sure if that would be required for AOS. Thanks for clearing that out.
