K-1 and adjustment of status medical exam requirement

So, when filing for adjustment of status after entering the US on a K-1 visa, there is a medical exam requirement.  My fiancee did her medical exam and received the DS-3025 indicating that she fulfilled and completed all the vaccination requirements.  On form I-693 instructions, it indicates " Even if a new medical examination is not required, you still must show proof that you have complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit Part 1, Information About You, Part 2, the vaccination chart, and Part 5, the Civil Surgeon's Certification, of Form I-693 (in an envelop sealed by the civil surgeon). "

 

How do I know if the vaccination records (DS 3025) was included as part of the original, overseas medical exam report? Because that report was sealed at the interview at the Consulate in Vietnam and we couldn't tell what was really in it.  All I know is that my fiancee has a DS3025 completed but that was outside of the sealed package.  Can't tell if that DS3025 is a copy or not.

If he has one that is a copy. Take a copy of it and include in the aos package so they IO knows that he has complied. Sometimes the medicals seem to not be linked to the A-file already, or the IO doesn't know to look for it if not familiar with K1's.

That seems to be her copy of DS-3025. I would make a copy of DS-3025 and send it to them along with other documents.

Any DS-3025 that you have will be a copy (the original made it's way to the embassy/consulate and should have been put in the "envelope" by them).

 

If the DS-3025 is marked complete (as you stated), include a copy in the package and you should be good to go.

 

There is no way of knowing if the original is in the file or not. That is the main reason for sending the copy.
