First fill out  DS 160 application online at https://ceac.state.gov/genniv/ . Pay the money, thus visa fees at GT Bank, schedule the interview at https://cgifederal.secure.force.com. You will need the reference number on the GT bank receipt in order to make an appointment for the interview as well as the reference number on the ds 160 confirmation page. Very important, make sure all your information are the same, the barcode info on the appointment confirmation page must match the barcode info on the ds 160 confirmation page otherwise you will be turned away on the day.once your appointment is confirmed, you have to print the confirmation page and then get your medicals done at either Akai or Holy trinity. You need the appointment confirmation page before u can get your medicals done. You also have to get the police report and off course gather all your supporting docs in prep for the interview. Our case was expired way before we had our interview as well.. my fiancé still got his visa with no issues.. that expiration pretty much doesn't mean anything as long as your packet is with the consulate at the embassy. So you will be fine
