I'm from the UK so might be able to send some light on things more for you! 

 

This is the website you need to apply on: https://www.acro.police.uk/police_certificates.aspx 

 

It says on their website it will take 10-14 days to process & this is what they say about validity, i've highlighted the most important part: 

 

"There is no set period of time for which the Police Certificate is considered to be valid. The Police Certificate is only truly accurate on the date of issue. Embassies, high commissions, consulates or other authorities will usually require you to produce a Police Certificate which has been issued less than six (6) months before your visa interview."

 

The embassy website is really useful, I'm going to use that a lot after NOA2 arrives. Tells you everything you need to know to prepare for your interview:

Yup I'm sure it wasn't but that seems to be one of the main reasons due to the fee change

Yup as I said above, there will be multiple reasons. But looking at the spreadsheet - there were a lot of applications rejected due to the incorrect fee
