Hey guys, I'm trying to get together all the documents for the AOS, but I'm not sure how to get the a vaccination supplement, I'd like to know what document you guys sent and what you did to get it, if it's the one I get from my past medical examination for the k1 visa, do I need to send a translation?

You will need to go to a civil surgeon here in the USA to get this vaccination supplement that you have from your doc that did your medical overseas (DS-3025) transcribed to a I-963a here in the USA. There are designated surgeons who do it here, but call around and MAKE SURE you are not getting the full medical done again, just the I-963a, because they tend to rip people off. Mine costed $5, but some have had it cost $75, so phone around.

Hello Sonia,

You need the I-693a Vaccination Supplement. This is all that you need. They will ask you for documentation of the measles, mumps, rubella (MMR), Tetanus (Td), and Varicella. I actually sent my husband to his primary care doctor to do a blood test to prove he had antibodies to this through my health insurance.

It was $25. I also called the one on St. Paul St and Knecht St from the list of Civil Surgeons. They also charge $25.

My wife sent the DS-3025 vaccination documentation worksheet from her visa medical exam. It was complete (she had all the required vaccinations that were mediaclly appropriate) and no I-693A was required.

DS-3025 is in English, no translation required. Unless the consulate you went through accepts a local version in the native language.

I just called all the offices for this area, and most are part of a network that charges the same fee ($45- not including any vaccinations that need to be done!).

Most offices are trying to tell me that I need the whole medical...I am a K1, so I know I don't....but I don't have a copy of the medical I got done in February, so how do I convince them that I only need the I-693A??
