What do you mean "the DS3025, is merely a copy of the full form"

It's a form in itself. The copy that she's suppose to have...is...merely a copy. The issue is if they have not included it in the package she most definitely will get an RFE. The case where she might not get an RFE is when they have completed it and included it in the package but you just don't have a copy to send along with your AOS application (not needed). In that case you can get an RFE if they have lost it or it's not filled in correctly. If it's not there at all to begin with the requirements are simply not met.

I'm meaning that the DS3025 the OP was looking for them to give her is just a copy of a portion of the documents that go into the envelope. What it sounds like to me is that the nurse knows that you don't need any vaccinations in order to receive the K1 visa, and are so lazy/ignorant of the rules they just aren't going to bother with doing the paperwork. Which is both partially right also completely wrong. They should be bothering with the form, even if the OP can still receive their K1. That being said there are other countries that do this to their patients. And yes, if it's not there to begin with for AOS the OP will have to go see a US civil surgeon, and have them complete part of the I-693. Which is why I said to be proactive on this, in making sure they had a list of whatever vaccinations they had received translated because if the foreign CS doesn't budge on this they will need to go to a US CS for AOS.

That being said there are other countries that do this to their patients. And yes, if it's not there to begin with for AOS the OP will have to go see a US civil surgeon, and have them complete part of the I-693. Which is why I said to be proactive on this, in making sure they had a list of whatever vaccinations they had received translated because if the foreign CS doesn't budge on this they will need to go to a US CS for AOS.

That makes more sense, and I completely agree :)

Thank you everyone for your messages!!

I am so confused. A friend who did the medical in the same place last year told me that the doctor gave him the vaccines records. Why wouldn't they bother to do them for me? I completely believe the lady was too lazy to bother, as it is not 100% required to get the visa. I have to say, I am from Spain myself, and this is something I really hate about my country: people are so lazy, and if they can avoid doing some work they will avoid it. Of course, it's not fair to Spaniards who aren't like this, but being from Spain myself I can say 90% of people are like that.

Sorry for the rant. I am just so annoyed right now! I'm not sure of what to do. I may go again on Wednesday with all this info and be firm with them about this. I mean I did pay over 300 bucks, they might as well do their job.

Try to get as many official looking vaccination records as you can and make sure they have an English translation. For anything that I couldn't get an official looking record of, I would go get it in the US (again) for as cheap as possible before seeing the civil surgeon and ask for documentation of it. Some pharmacies can do immunizations for not too much money (less than a civil surgeon will charge anyways), and any documentation you get in the US from a pharmacy or clinic will definitely be accepted by the civil surgeon.

Then, when you get to the US, make phone calls to the civil surgeons in your area and find out which ones know about the K-1 AOS process and know that you only need the immunization record, not the full exam. In my case, I called about 6 places and only found one who knew about this. Others wanted to charge hundreds of dollars, and one even tried to accuse me of fraud for asking whether they could provide us only the immunization page. I brought the combination of official looking foreign vaccine records and American records from the things we re-did in the US, and the civil surgeon was able to give us the page we needed for cheap.

Additional snafu: my fiancé came over on his visa, but just a month later (and the day before we were going to get married) he had an emergency that necessitated him returning to China.  I contacted the Guangzhou consulate via their email system, provided the requested documents/proof, and he was granted a second interview for "reissuance."  He had to re-do the medical, redo the interview (bringing yet another copy of all of the things he had to bring the first time) and we were approved again!  He received what looked like a new visa in his passport, but the original 90 day countdown for marriage was still ticking.  He came back to the US and we got married right away!

You know, in situations such as the one you are in now I like to remember the way the Dalai Lama philosophises about these things. See this (and ppl like that) as an opportunity for you to practice your patience :rolleyes:;)

I would be really upset at having to take shots again for illnesses that I am already immunized again, just because someone was too lazy to fill out a form. My wife and I are dealing with civil surgeon here in Arizona prior to her AOS package being sent in. The cost varies (depending on who you choose) but ours will be about $130. He won't be doing a complete medical checkup but just simply verifying all required tests and immunizations are accomplished.

Even without the vaccinations records, you can still get the required vaccinations, MMR, Tdap/Td, varicella(or history of chickenpox) and flu shot(only during flu season, which it currently is).

If they cost too much from the civil surgeon, you can get them from someplace cheaper, like your local health dept., and then take that vaccination record to the civil surgeon to transcribe them onto the I-693.

I feel your pain. Mexico doesn't typically provide K-1 applicants with vaccinations. My husband received a useless, blank DS-3025.

I just really hope she does her job correctly and gives me a blank waiver, as I am pretty sure I have all vaccinations that are appropiate for my age! Fingers crossed.

It was a photocopy for me and I guess otherwise you still don't know that the original is correct.. and for the aos only the one that goes with the package matters in any case.

Thanks! If the nurse does her job correctly and marks my vaccinations as complete, then I won't have to worry about this anymore, right? I won't have to do any medical or vaccinations for AOS.
