AOS G-325 name to be used at the bottom?

Timeline

We are married and we are filling out the G-325a for the AOS process. The example form has in the bottom black box his wife's maiden name but at the top of the form it has her married name. Do I need to use here new married name in the black box or the way it is in the example. Seems like it should be her married name and not her maiden name in the black box on the form.

I used my married name at the bottom. In fact it is the only name I use now because it is the on I want to go by. If anyone needs me to provide proof of name change I just submit my marriage certificate.

Married name top and bottom.
