My fiance is here on a K-1 visa. We are in the process of submitting the I-485 form (Register Permanent Residence).

Since she had her medical exam in Venezuela, it says all we need to submit is a vaccination supplement (which I believe is form DS3025). However, my fiance was given a SEALED envelope after her examination which she gave to the Embassy in Venezuela for her interview. She never got anything back in return regarding her medical report or vaccinations. She was given another sealed envelope from the embassy to give to the immigration officer in Miami at her Point of Entry.

How can we submit the vaccination information if she never received anything? Can we assume they already have this information for her since it was in the sealed envelope? Can we request a completed DS3025? I would hate for her to have to take ANOTHER medical exam and get ANOTHER round of vaccinations - that does not sound healthy to me.

Can anyone provide me with info about this? The DS3025 form says "give copy to applicant" but she was never given a copy.

Any help would be very appreciated. Thank you!

Some are given a copy and some are not. Some are given it upon request. The original DS-3025 is supposed to be in the enveloped handed over at POE. You are not required to have this (vaccination supplement) with K1 medical within one year if DS-3025 is filled in correctly. Without a copy you don't know if it is. Your options are to submit without, assuming everything has been done properly from the physician's and immigration's sides, or to pay and perform an additional medical (or just vaccination portion of it if you find a doc that will do this) and have a i-693 (or i-693 vaccination portion) filled out and sent in with your application.

If I were you I would send it in as is and do the i-693 if an RFE is received, but this decision is entirely in your hands :)

Everything SHOULD be fine as is, but as always things can get lost, ppl make mistakes..
