Help with Affidavit of Support for K1 Visa

I was trying to fill out the Affidavit of Support that I need for my fiancée's K1 Visa and I had some questions about it that I hoped people on here could answer. The questions are below, thanks in advance.

1. I do a lot of freelance work as a photographer, but also have a full-time job working at a thrift store. I wanted to put both types of income on the affidavit, so how would I go about this? The affidavit doesn't seem to have an area to enter self-employment information.

2. Relating to the above question, how would I go about gathering the evidence and information for my freelance/self-employment income?

The self employment income will show on your tax return.

If you note the photography work you would have to show you paid taxes on it. So past tax returns would be your evidence.
