Do we include I-683, if not what proves vaccine records for K1 AOS?

Hi -

I am a bit confused about what to include in regards to the vaccination records included in my K-1 visa. Do I include those previous records and/or Form I-683? Is Form I-683 necessary when applying for K1 AOS?

So, do I need to submit a DS-3025 form? I never received one.

No, it would have been included with the rest of your medical documents in the sealed envelope you handed over at your POE. You can include a note on your cover letter that your DS-3025 vaccinations worksheet was in the envelope with your other medical files, and should have been forwarded to the USCIS from your POE.

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.
