Hi there everybody,

My wife got her green card back in July of this year. Took a long time, but we are happy it got done. Now we looking to get enrolled for school. In order for her to enroll, we need to have a copy of immunization forms. We even went to the doctor's office and they said they needed to see a copy in order to determine what she has already had. This is the problem. Her father called the doctor's office in Vietnam and they said that they do not keep the records. So we cannot get a copy from them.

So what can we do? Is there anyway we can get a copy of the immunization form that we had to submit to the U.S. government when we she arrived on the K-1 visa? Would the immigration department help us with this? Any help would be great. Thanks.

Did you keep a copy of the DS3025?

Can you contact the medical facility in Vietnam where her medical for the K1 was completed to get a copy?

It is very difficult to get back or a copy of documents given to USCIS. I do know of one VJer who was able to but it took alot of work. Maybe try calling or making an info pass. It's not the easiest route though.

In the mean time get her in the doctors to get blood work (titers). Then they can give her a new record and if she is missing vaccines they will know and be able to give them to her.
