What did the notice say? What did you put in family name? Did you leave it blank? You may have to make your first name your family name, I have seen people with "First name unknown" but last name might be a required field. If you explain what exactly happened it will help.

AOS for my husband
8/17/10: INTERVIEW DAY (day 123) APPROVED!!

ROC:
5/23/12: Sent out package
2/06/13: APPROVED!

You also need to understand what a family name is.

My father and I share the same last name. That's our family name. Your parents and you should share a name.

What you may be confused on is the middle name. My wife didn't have a middle name...just a first name and a family name. So we put NMI in the middle name blank (no middle initial). When she naturalized, she took her original family name as her new middle name, and my family name as her new family name. So she didn't have a middle name growing up, but she has one now that we are married and she's naturalized.

Your parents names should probably be first name, NMI, family name on the form.

Regards,

Bill

i do have a family name GILL but my parents they did not put family name in their passport thats why i don't know . I know family name is a last name. on their passport is only says first name middle name and family place is empty .

The application/petition has not been fully completed .One or more of the following fields weren't completed.

The notice says .... Your Alien Relative's Family Name

thank you
