I changed my maiden name to my married name in the last month or so and it is now time to submit my I-751. Can I list my new married name on the I-751 form and include the relevant documentation relating to my name change (marriage certificate, new SS card, new ID, etc.)? Or is there some separate process to go through to notify USCIS of the name change?

Also, we are moving in about one month. Should I use my current address and submit the form now and then file a change of address as soon as we move? Or wait to file the form until we move so that I can use the new address? Card expires on 2/12/17.

Use your married name in 1a-1c, and your maiden name in 2a-2c. And the instructions for the I-751 require you to send copies of the marriage documentation. *DO NOT* send the originals. You'll take them with you to the interview.

I wouldn't wait for the change of address to file your I-751. If your card expires in February, you need to get that started. Use your existing address (the address at the time you are signing/filing the form), and do the change of address once you've actually moved. (Your change of address will have references to your individual case number to link it together.)
