pre greencard health insurance and overstay change of status:

I have a related question but am not sure how to navigate to beginning a discussion or if one exists so here goes.

What looked to be the worst case of jet lag the world has ever seen turned out to be morning sickness. One going away party too many, I am not the father. USDA certified virgin on pre K1 medical exam so to speak. Got her a ticket back home to Philippines to try to regain her health and reboot from there. Left her with an aunt in USA where pregnancy became apparent and she remained in USA way overstaying her visa. Reunited with Babbie's birth and been living together as best friends and fiances. Family concern legalese prenup marriage preparations well underway before Trump elected. Baby 18 months old and a US citizen. What additional barriers to 'completing the petition' may this entail? And given the delay between the change of status and Green Card faced by all, what are people doing to get covered for catastrophic medical insurance? especially with questionable legal status in our particular situation..
