I am filling out the g325 form and I have a question regarding the field pertaining to mother's family/maiden name and first name. My mother was born in Poland where she changed her surname (maiden name) after getting married. She was then naturalized in the US and she changed both her name and surname. As her name after naturalization is different from both her maiden and husband's surname I will fill the form with her maiden name, however should I include an attachment with an explaination (she was naturalized from a different last name than her maiden name)? I just want to make sure that if someone does a background check, they will know the reason for the different names.

It would not hurt at all to provide this additional info to clarify which is better than an RFE and having to explain it later.
