Separated filing for Citizenship

Timeline

Hello, please i need your thought on this . I'm filing N-400 based on 5 year rule.

i have been separated from my wife for 3 years now but not legally, just physically separated.

my questions are:

1) Do i need to fill in her info on the document or just leave it blank ?

2)Do i need any supporting document to send to them ? like the marriage certificate ?

what i have so far

N-400 filed

Money order for $680

2 passport

Copy of GC front and back

do i need any other thing ?

your thoughts will be highly appreciated

For those filing form N-400, please note that the USCIS has a new form dated 09/29/16. That form comes with new instructions dated 09/29/16.

One item to pay attention to is that the list of required documents has changed slightly.

For example:

1) A copy of your legal marital status is now required even if you are applying under the 5 year residency requirement.

2) Photographs are now required only for applicants who reside overseas.

What is confusing is that USCIS still has the "old" "document checklist" file up on their website listing the old required documents. It is dated 01/28/2016.

It also looks like the visajourney.com people have not updated their guide section.
