Couple of questions about filing for AOS

Hi,

Just about to send off my AOS packet. Couple of questions I'm a little unsure on and would appreciate any guidance.

Sending off the medical/vaccination form. I also got a flu shot at the weekend. Should I send that in as well, as we're in flu season?

Photos. I have 2 copies of the same passport photo, I've now seen that some people recommend sending 6 for EAD and AP. Does it matter if all the photos are not the same?

I'm in PA, which is the correct address to send the packet?

Lastly, the check/money order. Which is best or does it not matter? Also, who should it be made payable to?

We just sent my husbands AOS packet today. He had a few passport photos leftover from his last interview, but not enough so we went last night got all 6 matching ones. Not sure if that was necessary but didn't want any issues.

Sure send in the flu shot record. It's not on the DS3025, but it can't hurt to send it in. If they need more they will let you know.

Check for $1,070 was made payable to US Department of Homeland Security. Same as the I-129f peition one.

If sending by USPS, it goes to the Chicago Lockbox address. If sending by Fed Ex use the physical address.
