Letter of Intent to Marry

I am about to send in my K-1 paperwork for my fiance and I. Here is my question. I sent to him via DHL several forms to sign. I got them back this past week. On the "Letter of Intent to Marry" he signed the form this way:

Signature: ______First name_________________

Print name: ________last name_________________

I wanted him to sign the top line and print his name on the bottom line. Instead, he put his first name on the top line and his last name on the bottom line instead of signing his first and last name on the top line and and printing first and last name on bottom line. Is this going to be a problem or do I need to send him another form and have him sign and print it the correct way? We just hate to spend more money on DHL fees, but at the same time want to make sure everything is done correctly. TIA!

I don't know the format your fiance used in writing it, whichever way he chooses, the Intent to marry within 90 days of his arrival must be included and has to be signed and dated with his names as shown below
