Received our 2nd rfe today.

In short it states they want a 693 completed by a surgeon, signed and placed in a sealed envelope. Only requesting page 7, vaccination records.

I completed my medical in Manila, on April 2016. I have a 325 (?) That lists all the vaccinations i received. Signed by the doctor. The box stating all US vacation complete is NOT checked.

We called the 800 number and assked if we could use the 325 as proof. The woman wanted to transfer us to a tier 2, but was unable. She advised us to call Monday and ask a tier 2.

Anyone have recent experience with this situation? Will they accept the 325, or should we just schedule a appointment with a civil surgeon.

Thank you for any help you can provide.

On the bright side our case was transferred to Virginia.

Take the DS3025 to the US Civil Surgeon of your choice. You are going to have to find one willing to just do the vaccination transcriptions only, so it may require some searching. It's not just page 7, there are a few that will need to be completed (but NOT the full form). Refer to the instructions for the I-693 for the correct way to complete it. You'll fill out some stuff and the doctor will do the rest, it will be placed into a sealed envelope and marked. That's what you'll send in with a copy of the RFE and a cover letter. Once you respond to this RFE, you should be good to go.
