Hi, my name is Daniel. My fiance is filling out police certificate form from Bahrain. She lives in Philippines now. She lived and worked in Bahrain last year for over 6 years. On the form in the address section it asking for address.

Does she put her address where she lives now in Philippines??

Or address where she lived while she lived and worked there?

Here is pic of form

Hi I am currently trying to fill out this same form. It is quite confusing.

Can you tell me how your fiance completed it? Did she use her current address or the address in Bahrain?

And did your fiance get her clearance certificate?

I lived in Bahrain 10 years ago. So I have the added difficulty of trying to find my CPR card and number!!

Many thanks for your help.
