Question about my birth certificate before K1 interview!

Hey everyone, my k1 interview is scheduled for November 30th in Montreal and I have a few questions:

My birth certificate is the original from when I was born and so it's very worn, when I went to make a copy of it today I noticed that the last number of the certificate number is faded and I can't make out if it's a 6 or an 8. Is this going to be a problem? I see that I can get a new birth certificate made in 5 days if I pay for the premium service...should I do this? I'm very worried about it

Just as an aside, the copies they ask for can be in black and white, correct? Also, I obviously got the email for my packet 4 but does a physical copy also come in the mail?

After looking a bit I am now panicking. I never knew there were two types of birth certificates in Ontario. The short form and the long form? Gahh. Do I need the long form for my interview?

I don't know how it is in Canada but the Embassy in Dominican Republic only accepted the long form.

It is highly likely you are going to need the long form also.

Get the long form version (certificate of birth registration), I'm pretty sure the small one is not sufficient. You need to fill out as much information as you can about your birth on the Service Ontario website. I paid for premium service and had mine in two days, well worth the money. My mom didn't remember a whole lot, so I had to call the hospital I was born at, and get one of the birth registrars to go into the basement and get my chart out of a box!

You need to fill out as much information as you can about your birth on the Service Ontario website. I paid for premium service and had mine in two days, well worth the money. My mom didn't remember a whole lot, so I had to call the hospital I was born at, and get one of the birth registrars to go into the basement and get my chart out of a box!

gah this is so stressful but thank you for your help, I'm trying to fill out the information now but this website is the absolute worst. I have two questions that you can maybe help me with since you've done it! The hospital I was born at still exists but it goes by a different name, the drop down list of options gives the new name...should I just use it or list the old name? Also, it wants the address of the doctor who delivered me? do they want his current address or the place he was at when I was born? WHY DOES THIS FORM HAVE TO BE SO SPECIFIC??? It's killing me...I would call the service Ontario place but the call centres are closed right now

So I should only order the long form version correct? I don't need to order another version of the smaller one just because it's worn since the long form should have everything I need?

I know it seems daunting at first-however when I called Service Ontario they said to just fill in as much as you can. Maybe if there is a section to fill in the name of the hospital, put the old name as well. For the address of the doctor, just use the address of the hospital you were delivered at. Chances are, someone to be born at the same hospital as you, at the same time, with the same doctor, and same parents (lol) is pretty slim ;). You should be all good!

The long form will work for everything so you do not need the short form anymore. Also you don't need a copy of the birth registration, just the birth certificate.

The long form birth certificate in Ontario is a certified and sealed copy of the Birth Registration. That's why i used the term, but they're both essentially the same thing (long form and birth registration).

I had just renewed my passport after having one for 5 years and they didn't even ask for my birth certificate, so I didn't even realize that there would be an issue until today :( thanks for your help! I'm working on getting the long form certificate now.

After you're married, if you want to change your name on your passport, you need to file for a whole new one, vs a simplified renewal like you just did.
