Hi



I have my medical this week, I have all the required evidence and vaccinations but there was one part of the questionnaire which I just want o get a second opinion on.



The section which ask: Prior Country. I was last staying in the states on a tourist visa from Autumn 2011 until Summer 2012. I know this sounds like it should be simple but I don't want to get it wrong. Should I put 'US' in this section. I technically wasn't a registered resident, so was a little unsure if I should state either 'US' or 'Not applicable'.



Also, In the section which asks: Have you ever been hospitalzed? I haven't ever spent the night or been rushed to hospital etc but have suffered from Asthma issues/Minor attacks resulting from chest infection, 15 or so years ago but was only in hospital for a few hours. so do I mention this or just put no?
