RE: I-751 JOINT PETITION TO REMOVE CONDITION OF STATUS

CONDITIONAL RESIDENT: WIFE

US CITIZEN SPOUSE: HUSBAND

Dear Sir or Madam:

Enclosed please find;

A.

· Letter request to update or change the last name

· Copy of the new passport

· Copy of our marriage certificate

· Copy of our Report of Marriage

· Completed I-751 form

· Application and biometric payment in check $590.00

· Copy of the Permanent Residence card, front and back

· Notarized Affidavit from my mom, Maria Doe stating that my wife(LPR) and I lived with my parents for a while before we bought our home at 1234 ########### in 02/2016

As evidence that our marriage upon which we were granted conditional status was entered in “good faith”, we are in addition enclosing the documents listed below;

· Copy of jointly filled 2015 Tax Return Transcript and Tax account

· Copy of our house deed, both in our names

· Copy of monthly mortgage payment coupons, both in our names

· Copy of our ******* house insurance, both in our names

· Copy of our old and new driver’s licenses showing the previous and current addresses

· Copy of our car loan and buyer’s order

· Copy of car loan payment coupon, both in our names

· Copy of our Ford Escape property tax 2016,both in our names

· Copy of vehicle registrations (ford escape), both in our names

· Copy of auto insurance cards for vehicle ford escape, both in our names

· Copy of auto insurance cards for vehicle Jetta, both in our names

· Copy of my wife’s ********** old and new DEER’s identifications cards and my military identification card to show that she is my dependent (I registered her since July 02,2014 before she became an LPR)

· Copy of health insurance summary explanation of benefits

· Copy of dental insurance cards

· Copy of dental insurance explanation of benefits

· Copy of a dental bill from our previous dentist, showing both our names

· Copy of ******** bank credit card (authorize since 09/2016) ****I added my USC husband as authorize user

· Copies of the monthly statements of our savings in Bank of *****.

· Copy of our *********** bank check, showing both in our names

· Copies of monthly statements of our savings and checking in ********* bank - My mom, ********* is listed on the checking account due to my previous deployment, I authorized her to take care of my financial responsibilities while I was gone then I added my wife ****** when she arrived. My wife and I decided not to remove mom just in case we need her to run our bank errands.

· Copy of the Schewel furniture loan and payment receipts

· Copies of our power bills

· Copies of our (gas) energy bills

· Copy of our dog’s Certificate of Vaccination, shows both our names as owners.

· Copy of our dog’s patient’s chart from the vet, showing both our names as owners.

· Copy of our ********* bank check, showing both in our names

· Copies of our ************ bank joint checking account monthly statements/transactions. – My mom, ********* is listed on the checking account due to my previous deployment, I authorized her to take care of my financial responsibilities while I was gone then I added my wife ******** when she arrived. My wife and I decided not to remove mom just in case we need her to run our bank errands.

· Copies of our ******* credit cards, showing that my wife, ******* is an authorize user.

· Copies of the ******** credit card monthly bills (only primary holder’s name appear on the bills but my wife is an authorize user)

· Copy of another ********* bank check ( added on 10/06/2016) - I added my USC husband to my checking

· Verizon mobile monthly bills – showing two cellphone lines under my name (wife’s # ************and my # ***********)

C.

· Copy of military life insurance beneficiaries

· Copy of 401K beneficiary designation - I added my USC husband as my primary beneficiary

D.

· Copies of our Lifetime Wellness Center membership cards

· Receipts of gifts purchased for my wife (Valentines 2015 and 2nd year of our wedding anniversary)

E.

· Pictures of us together

· Pictures of us with family and friends

We are both happily living together as husband and wife. My wife and I request that this petition be accepted for removal of condition on the residence of ****************

Note: This is what my USC husband and I came up with. I am worried and nervous and what address should I send my packet? I am sending it to Vermont, I am not sure if I should just put the P.O box number or should I also include the St. # and name?
