no matter how u filled out on visa application, the us consulate go with vn custom name format. what interesting part is that when your spouse arrives at the airport, the us custom does not know the recent consulate change format and choose spouse middle name as first name. It happened to some people since last year.

This glit has to do with inconsistency between consulate and us customs border

So I'm not exactly clear yet. Her visa and passport is in the Vietnamese format. Last middle middle first. For the airplane ticket, entering it online, her first name is still first and her middle is still middle on the form correct?

I just bought ticket through travel agent for my mother going to be here as a tourist. On visa and passport her name is in vn format even when I filled out DS-160 I went with US format. It goes as Hoang Danh. Hoang is Middle and Danh is first name. Travel agent here is US also went with vn format. Surprisely when I checked the ticket with AA.com it showed as Danh is middle and Hoang as first. Just don't know why, I going to give an agent a call and get back with you.

What I had concern is that when your spouse will arrive here the Border Control will give your spouse middle name as first.

I have an issue when I fill out form DS-160, US visa application. I think someone can help me out. On Vietnam passport, at "Place of Issue" (the passport), they print the name of the goverment agency that issues passports: "Cục Quản lý xuất nhập cảnh". It doesn't show anywhere the name of the city in which my fiancce made her passport.

On form DS-160, they ask "City where issue" (the passport). So, to answer this question on form DS-160, do I fill in "Cục quản lý xuất nhập cảnh", or the name of the city in which my fiancee really made her passport?

Thank you for all your feedback.

to Danthong,

Place of issued: city and state, just put down city and province where your spouse applied for passport. In case of Ho Chi Minh, just input Ho Chi Minh for city. For state input check mark in the box for "no state or something like that"

Sorry I can't help you with this. I put "document type" other. The wife didn't have a passport then.

I just called travel agent and was told that you must go with the format what is on the visa and passport.

  • 1 year later...

Mình đang phải renew US visa (non-immigrant) và rất băn khoăn trong việc điền DS 160. 2 năm trước 2016, 2017 đều điền First Middle Last, nhưng sq Mỹ cấp visa điền Middle First Last. Sang Mỹ khi làm thủ tục bị nói là khai tên sai, phải là First Middle Last.

Giờ thấy confuse quá. Có bạn nào mới khai DS 160 tháng 7 2018 (vừa xong) cho tí thông tin update nhé. Tks
