hi everyone,

my wife is an austrian citizen who will be coming to the US this may after she completes her studies. she has already obtained a K-3 visa.

we plan to file for adjustment of status as soon as possible after she enters the US.

she already had a medical exam and vaccination check in vienna, just a few months ago. (everything was fine, she is up to date with her vaccinations) we received a sealed envelope from the consulate.

i am looking through the I-485 and trying to determine what additional medical documentation will need to be submitted in the states, if any.

i emailed the vienna consultate this morning and they wrote back the following:

"Please be advised that a copy of the medical report form DS-2053, Medical Examination for Immigrant or Refugee Applicant and the form DS-3025, Vaccination Documentation Worksheet were enclosed in the x-ray envelope, which was returned to your wife on the day of the interview at our office.

As this office advised your wife, she must present the passport containing the K-3 visa and the sealed packet to the Immigration Officer at the Port of Entry (POE).The officer at the POE will keep the sealed envelope. "

do i still need to go with my wife to a civil surgeon in the US to file the I-693 vaccine supplement? we do not have a second copy of ds-3025 (it is in the sealed envelope from the consulate) but i do have a list of her previous vaccines that has been signed and stamped by a vienna clinic.

any suggestions would be most welcome!

cheers

If you send in the applications for AOS before one year has passed since her medical exam, she will not require a second exam. She may need to complete the vaccinatoin supplement however with a civil surgeon in the US. Just take the vaccination summary from the first exam to a civil surgeon and they'll take your money and fill in the form.

If she kept a copy of her vaccination sheet from her medical back home, and if you submit your AOS before one year has passed, you can just get a I-693A vaccination supplement with a USCIS civil surgeon using the record from her country, and send only that instead.

Some people have sent the actual vaccination sheet from abroad without issues as well.

Thanks for your reply.

The problem is we never got any vaccination summary ourselves -- this is all sealed in the official envelope that my wife apparently has to hand over as soon as she lands in the US.

Some people managed to get a I-693A supplement using the vaccination cards from the foreign country, you should call around to see if any civil surgeon is willing to do that.

Ok, Glad I am not the only asking the same question. We went through Manila and the medical vaccination record is on DS-2053, not an I-63 or I-63A. However, the Honolulu POE takes possession of the X-Ray folder and all contents inside. I am in the military and think I can get the Military Doc to transfer the vaccinations to the I-63A, but if not, will the DS2053 suffice when filed with the AOS?

Thanks ken
