Very glad I found this site. I am just getting ready to mail my AOS package off and would like to

ask a few questions before I send it away. Thanks for your quick responses in advance.

1 G-325A: Last address abroad for more than one year, is this my last address before coming here?

2 G-325A: Family name (for wife give maiden name), is this the name from deceased spouse or my

original maiden name before ever been married?

3 Separate checks for AOS and Biometrics ($395) or I can write just one check or it must be money order?

4 US citizen spouse's passport, biographic page or whole passport including front and back?

I know these are silly questions but just want to be sure. Thanks

Hi welcome to VJ!

I can answer questions 3 and 4 based upon my experience:

#3 We sent one check for the biometrics for $395.

#4 I don't think you need to include the paspport for the USC citizen spouse (at least we didn't).

But I am sure that if I am wrong someone will come along to correct me. And hopefully someone will answer your questions 1 and 2.

#1 - exactly what it says. Was your last addy for more than one year, if so, put it.

#2 - are you a woman or man? Use whatever name you are currently using. Under other names used, put your maiden name.

#3 - $395 - Use a money order so you can track the payment. If a personal check is allowed, use that instead.

Question #2 I am a woman, I was married before, it's asking for maiden name, before I got married to my present husband I had my deceased husband's last name.....should i use that as my maiden name or my original name b4 I was ever married?

Hi Torsans. I think you're mixing up two separate questions. The first field requests your current family name. Then you enter any previous names used, including your former married and maiden names. Later on it asks you for your former HUSBAND's name... not your name.

Question#3 some people say send separate payment, can i just sent it all together ($395) for both AOS and Biometrics and does anyone know if personal checks can be used instead of money order?

The instructions say you can send a personal check or money order in the amount of $395 for both the I-485 and the biometrics fee. You do not need to send a separate payment. (See Page 6 of the instructions to Form I-485.)

107 days from mailing AOS package to receiving green card! Received 6/27/07 dated 6/20/07

Removing conditions:

mailed 5/22/2009

Recieved at CSC 5/23/2009

Check cashed 5/27/09

NOA date 5/26/09 - still not received

I-551 stamp in passport 6/19/09

Biometrics notice date 6/16/09

Biometrics received 6/20/09

Biometrics apt date 7/9/09

Biometrics cancellation notice 6/27/09

Biometrics apt notice date 6/26 recieved 7/7

Biometrics apt 7/22/09

Finally able to see online info 7/22/09

Card production email 8/19/09 date ordered 8/18

Recieved approval letter in mail 8/19/09 date 8/13!

Husband joined army and moved from San Diego to Ft. Benning GA! 8/30/11

Early Biometrics 10/04/12

In line for interview 10/8/12 for 8 days

Interview 11/20/12 PASSED!

Let me give an example. Befre i was ever married my maiden was was KENT, then I married and my last name changed to ROLLINS, then my hubby died..now i am married again to a USC and my last name now changed to LEHMAN. Based on this example is my maiden name my original maiden name (KENT) or the last name of my deceased hubby (ROLLINS)? I know I am being retarded (hehehehe) but just want to be sure. Thanks again

I might be mistaken, but I think somewhere either on the G325A or the I485 you are asked what other names you went by or are going by at the moment. I think I would use your maiden name, i.e., the first family name you had. Then list the other names you have had.

Your maiden name is your FAMILY name... Therefore in the example you gave, your maiden name is KENT. It is your "birth name" as it known by some.

I dont remember which forms, but some will ask for other names, then you would include ROLLINS or whatever your name was when you married the first time.
