Hi all,

I came to America on IR-1 (thanks to the help of VJ for that process) on 09/05. My Greencard has my maiden name on it since at that time I was not able to change the name on my passport. After I came to amercia, I applied for my SSN. hubby asked the lady if I could put my married name on it. The lady ended up putting my maiden name as my middle name and my married name as my last name.

Also, to make my situation even more complicated, I became a Canadian citizen after I got my US greencard. So strictly saying, my chinese passport is not valid anymore. However, it is such a pain in the neck to appy for Canadian passport, that unless I absolutely have to, I don't want to go through the trouble applying for one.

So here is my situation:

Chinese Passport in maiden name

Canadian citizenship card in married name(no midden name)

Greencard in maiden name

SSN in married name(with maiden name as middle name)

Drivers license in married name(with maiden name as middle name)

I'll have to apply for lift of condition this June. Can someone please tell me if I should put my maiden name on I-751, or my married name, and what other documents I need to submit together with my application for the name change?If I put "Canadian" as the "Country of Citizenship"? Will they require supporting document? Will my citizenship card be enough?
