no, it is good from the date of he medical. And no, the medical is not good for a year, it's good for 6 montths

You can likely push back the interview OR she can enter and return to finish schooling.

Like this poster said, wouldn't the simplest solution be to go through POE (say after 4 months), stay a few days in the US, then go back for the couple of months need to finish study and afterwards move to the US permanently? There would be an added benefit that physical green card and SSN production would be in the works while finishing studies.

I suppose the only drawback would be the cost of extra flights, which might not be so easy for a student to fund.

This is what my wife did as well. She touched down in 2014 spent 3 days and then went back to med school for another 6 months before she returned to the states. I do not think that USCIS expects everybody to be able to just drop their lives outside so quickly.

Something which may be worth bearing in mind is that provided you departed on or before the expiry date of the immigrant visa then it's considered valid, even if it takes you longer then one day to travel to the US.

Suppose your immigrant visa expires on 1st April. For some (weird) reason you depart on 1st April and don't actually arrive until 2nd April. Your immigrant visa is still considered valid because you departed on or before the expiry date of the visa.

8 CFR 211.3:

"An immigrant visa...shall be regarded as unexpired if the rightful holder embarked or enplaned before the expiration of his or her immigrant visa".

My 2 cents: your best bet would be to reschedule her interview date date for a later date. This may get pushed back by few months but that's what you would want, right? If her medical test results gets expired due to delay they will tell her to do it again. Would that hurt you financially much? If not, dont worry and do it again.
