Are you saying you got an RFE for not showing proof of vaccinations?

You need to prove to a US Civil Surgeon that you have been vaccinated. This can be accomplished a number of ways.

If you have form DS-3025 from your consulate medical exam, that is helpful. If you have vaccine records from your home country that is helpful.

You can elect to have a titre test performed over here which will show immunities you possess from prior vaccines or illnesses. That is helpful.

When i got here in US my husband brought me in the doctor not related in civil surgeon.. when i submit my all documents for AOS i did submit also my record from my country and also my record the doctor if where i went to..but i got still RFE that they need sealed enevelop from civil surgeon.. what i need to do now? anybody can help me...

You'll need an I-693A VACCINATION SUPPLEMENT FORM completed by a USCIS listed doctor in a sealed envelope. http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf bring also your vaccination records from St Luke's.

If you require a medical exam (after 1 year since St Luke's exam), you'll need all 4 identical pages of I-193 form: http://www.uscis.gov/files/form/I-693.pdf also completed by a USCIS listed doctor in a sealed envelope.
