Last July, my son and I are scheduled to have an interview at the Calgary office, BUT, I rescheduled it in Nov. because I want my passport to be renewed first. (I should have renewed my passport first but, I got excited applying for the US visa lol)

Anyway, my passport now has my married name and the DS160 form is under my maiden name. Do I need to fill up another DS160 form? and Of course the passport number is different from the old one. What will I do? Please advise.

This happened to me also, no you don't need to reapply because that document cost money, when you send your marriage license and copy of documents they will see the name change, I always use my maiden name because I started the process before my new passport and other papers came. Don't worry just always use your maiden name when calling any agency, the name you filed the papers in.
