I'm currently compiling all the required documents for AoS filing, however I did not receive a I-693 after my medical examination in the UK (Kightsbridge Doctors). I was however given a US Department of State Vaccination Documentation Worksheet (1405-0113 for use with DS-2503), will I require to visit a doctor in the US in order for them to complete an I-693 form? If so, has anybody got any experience of doing this ideally in the Austin, Texas area?

The I-693a can only be filled out by a civil surgeon in the US. Take the worksheet to got to one in Austin and they will do it. You may have to call around for one first who will just fill out the I-693a and not ask you to do the whole medical again.

Feb 25, 26 & 28 - got emails saying that the cases had been transferred to another office, then to my local office, and then just transferred and are being processed

Mar 17 - got email, attached to one case number only, saying that my A number was changed relating to the I-130 filing

Maybe.

My wife and some others have successful submiting the D-3025 vaccination sheet from the visa physical, but others have been required to submit the I-693A vaccination supplement. So you won't know what's going to happen in your case until it happens.

You'll surely manage to find one, Andrew, just like I did. I personally would advise you to follow this route: take the vaccination worksheet you received at the medical in the UK to the civil surgeon so he/she can transfer the information on I-693a, then attach this latter document, which you will receive in a sealed envelope, to your I-485 application. You'll eliminate most of the chances of getting an RFE for it. As far as the civil surgeons are concerned, yes, most of them will try to talk you into having the whole medical done again, but if you are persistent, you'll find one who has read and accepted the USCIS instructions on the AOS application form and who will just prepare the Vaccination Supplement for you. Don't be ashamed of telling them on the phone, before getting to their office, what exactly it is that you need. Also ask about the cost: it shouldn't be more than 25-40 dollars if your vaccination history is complete. Wishing you best of luck, Callina

We had no luck submitting the Vaccination Worksheet and got an RFE for the entire medical all over again. We sent a duplicate medical ($100) and are now waiting to see if that satisfies them.

Just to be on the safe side, we'll get the I-693A (supplement) and send that too even though they didn't ask for it. Paranoid? Yes, Because others have been denied over this issue. Stupid but true. :blink:

Make sure you do call around though, some don't seem to understand you've already had the medical and say you have to have the medical again which isn't true. Also, prices vary, one place for me said they'd do it for $50, another for $10....guess who I chose? :lol:
